Physical Therapy, Athletic Performance (PTAP) & Sports Rehabilitation services for individuals of all ages! Insurances are not health care providers, so why let them guide your care? PTAP Sports Rehab, LLC was founded in Albuquerque, NM in 2021. It does not matter if you play a sport or not, because we consider everyone an athlete! Whether you have been limited in physical activity due to pain or injury, we will help you identify what you need and guide you in your journey to reach your true potential! Treatments can range from soft tissue mobilization (muscles/tendons), joint manipulations, sports massage, condition-specific exercise prescription and therapeutic modalities. Patient education regarding diagnosis/prognosis, postural modification, body mechanics, nutrition, stress management is an integral part of every treatment plan. This is why we include a foundational fitness and nutrition guide with every initial consultation to provide you with the tools for early success. There is commonly a lack of continuum of care in majority of Physical Therapy clinics today, however, we promise this is not the case in our facility. Our Physical Therapists (PTs) work closely with our highly-specialized personal trainers to ensure continuum of care is the forefront of our patients focused plan of care following PT discharge. Our experienced, licensed and board-certified PTs are rehab specialists making them both passionate and confident in their ability to help you achieve relief of acute or chronic pain, improve recovery time following an injury, and maximize your functional potential! Anthony Chavez, PT, DPT, LAT, ATC
Owner / CEO of PTAP Sports Rehab
Sports-Med Physical Therapist / Doctor of Physical Therapy

Dr. Chavez obtained his Bachelors in Athletic Training from the University of New Mexico in 2018. During his time at UNM, he worked with the sports medicine team for Lobo basketball, football, and baseball. He has also had the opportunity to work with USTA and USA Track & Field. A board-certified Athletic Trainer is highly-skilled in injury and illness prevention, clinical evaluation and diagnosis, immediate and emergency care, treatment and rehabilitation, professional health and well-being.

 

Dr. Chavez completed his Doctorate of Physical Therapy at the University of New Mexico where he graduated in July 2021. He had the opportunity to work in the intensive care units at UNM Hospital, and more recently, at Athlon PT “Movement for Life” clinic in Arizona. He worked with pre and post-operative patients as well as neurological populations such as patients with Parkinson’s disease. It was in Arizona where Anthony learned color boxing and hopes to continue implementing it into his training and rehabilitation. Color boxing is for individuals of all ages and skill levels looking to improve cognition, coordination, balance and stability.

Jennie is from Sandia Pueblo and graduated with her Bachelors in Athletic Training from the University of New Mexico in 2020. She is currently pursuing her Masters degree in Health Education with a focus in Community Health. Jennie has a variety of experience working with athletes of all ages in various sport settings as well as with youth development and academic success programs. Jennie’s work is directed towards health equity and equal opportunity for athletes of all backgrounds. She currently works for the NB3 foundation creating functional mobility programs for youth of all ages for soccer, cross country and golf.
